Disability Claim Forms
If you are off sick for more than 14 days, please make sure that your employer has submitted an initial disability claim advice form to our office so we can begin the application processes
Please do note that the other required documentation is submitted by your Human Resources Department

Death Claim Forms
Please note that if the employee was still working for the municipality before they passed, the family needs to start the death claim application process at their HR department
If the employee had already retired please contact us at our front office and submit the death certificate either in person or via email at statements@kznmpf.org or info@kznmpf.org
